How to Become a Primary Care Patient at our Clinic: 
Thank you for your interest in wanting to become a primary care new patient at our clinic. If you have seen another provider at Rockport Physical Medicine PA, you will still need to complete the application process for primary care services in our office. The Application includes the submission of a copy of your Photo ID and the Front & Back of your insurance card. Please take a picture of the front of your drivers license and the front and back of your insurance card before you fill out the application.

If you are wanting physical medicine, bio-identical hormone therapy, or aesthetics services - please call our office directly at 361-729-2225. No application necessary.
