If it is after hours and you would like to submit an application you can do so below. The Application includes the submission of a copy of your Photo ID and the Front & Back of your insurance card. Please take a picture of the front of your drivers license and the front and back of your insurance card before you fill out the application.
If you are wanting physical medicine, bio-identical hormone therapy, or aesthetics services - please call our office directly at 361-729-2225. No application necessary.
